Acceptance Guidelines

 

ADMISSION OF NEW STUDENTS
 
Saint Andrew School has a non-discriminatory policy in accordance with the guidelines of the Archdiocese of Chicago. The School does not discriminate on the basis of race, color, sex, creed, national or ethnic origin. 
 
Acceptance Criteria
 
Before a child may be enrolled, a completed application form is submitted to the School office. Applications are accepted from October through February for admission to our Early Childhood Program for the following academic year. Admission to Grades K – 8th is based on the availability of space. The following is the order of priority for admissions:
 
¨       Students currently enrolled in Saint Andrew School
¨       Brothers and sisters of children currently enrolled at Saint Andrew
¨       Children of parishioners seeking enrollment for the first time or now reaching school age
¨       Children of non-parishioners wishing to enter early childhood program, kindergarten, or first grade
¨       Transfer students from other Catholic schools
¨       Transfer students from public schools or non-Catholic private schools
 
Age Requirements
 
¨       A child entering the Early Childhood Program must be three years old by September 1 of the entering year
¨       A child entering Kindergarten must be five years old by September 1of the entering year
¨       A child entering the First Grade must be six years old by September 1 of the entering year, in compliance with the requirements of the State of Illinois regarding age of admission

 

ADMISSION OF TRANSFER STUDENTS
 
Saint Andrew accepts transfer students based on previous school records and behavior performance at the discretion of the Principal. To assure the child’s adjustment to Saint Andrew School, his or her first year will be probationary, based upon behavior and academic progress. The student’s progress will be reviewed on a quarterly basis by the principal and teachers. For admission of transfer students, parents will present a letter of notification from the school previously attended. Upon acceptance at Saint Andrew, all academic and health records from the former school must be transferred.
 
 
Documentation Required
 
Upon acceptance for admission to the Early Childhood, Kindergarten or First Grade student, parents/guardians will submit:
 
¨       Original copy of the child’s birth certificate. A copy will be made at school and the original will be returned. Hospital records are not acceptable
¨       One copy of the Baptismal Record, if applicable
¨       Record of physical examination and immunization history, signed by physician and parent/guardian. Forms are available in the school office
¨       Dental examination form filled out by a dentist
 
Immunizations
Illinois State Law requires proof of immunization for each student from the following diseases: Diphtheria, Pertussis, Tetanus, Poliomyelitis, Rubeola, Rubella, Mumps and Chicken Pox. All Kindergarten and 5th grade students must have a Hepatitis B Vaccination.    Please notify the school office, in writing, of any immunization updates.
 
All students entering school for the first time, Early Childhood, Kindergarten, and First Grade must provide proof of a current physical examination (within six months), immunization history, and dental records in accordance with Illinois State Law. In addition, all fifth graders must have a complete medical examination within 6 months prior to the beginning of the school year. Proof of these immunizations and tests must be on file prior to the beginning of the school year. These records are kept in the student's file in the school office. Non-compliance with the law requires exclusion from school. 
 
Book and Archdiocesan Registration Fees
 
A non-refundable Archdiocesan and book registration fee of $225.00 for Pre-K and $225.00 for K-8 is required. An Early Bird discount of $50.00 per student is available to returning students when paid by the March 1, 2009 deadline.
