Report CardXXX The writing sample, math assessment will be completed at Saint Andrew School and should take about one half hour. The interview will also take place at the school, following the assessments. Please call the school office to schedule an appointment.
Eighth graders are considered on a case by case basis, when exceptional circumstances warrant.
Documentation RequiredUpon acceptance for admission to the Early Childhood, Kindergarten or First Grade student, parents/guardians will submit:
ImmunizationsIllinois State Law requires proof of immunization for each student from the following diseases: Diphtheria, Pertussis, Tetanus, Poliomyelitis, Rubeola, Rubella, Mumps and Chicken Pox. All Kindergarten and 5th grade students must have a Hepatitis B Vaccination. Please notify the school office, in writing, of any immunization updates. All students entering school for the first time, Early Childhood, Kindergarten, and First Grade must provide proof of a current physical examination (within six months), immunization history, and dental records in accordance with Illinois State Law. In addition, all fifth graders must have a complete medical examination within 6 months prior to the beginning of the school year. Proof of these immunizations and tests must be on file prior to the beginning of the school year. These records are kept in the student’s file in the school office. Non-compliance with the law requires exclusion from school. Book and Archdiocesan Registration FeesA non-refundable Archdiocesan and book registration fee of $400.00 for Pre-K and $400.00 for K-8 is required. An Early Bird discount of $250.00 per student is available to returning students when paid by the February 1, 2011 deadline. |
